This form will be kept on file with 4-D Studios, Inc. and will be used to contact Agents with employment opportunities and other important information for their Actors. Please ensure that your Biography on file is updated as necessary. You can post your biography form here!

That's it! Don't forget to make sure your Actors fill out their Actor Biography Forms and turn them in to Casting Office!
Notes to the Player
This is fairly self-explanatory, but we'll explain it anyhow -- when joining the game, you need to fill out this Agent Biography Form (basically, your contact information). You will also need to fill out an Actor Biography Form for each Actor you want to represent. Agents (again, that's you) have much less strenuous requirements for their jobs:
Agent's Name: That's you. Your name is what other Agents (and The Director) will call you.
Agency Name: What you want to call your Agency.
Age: How old you are. You need to be 18 years old or older to join Shooting Stars.
Roleplaying Experience: Everyone is accepted here at Shooting Stars, so don't worry if you're inexperienced. Just give us a general idea of what to expect here.
Journal: The journal name you'll be using for your Agent/player-related postings.
AIM: Your AIM name, for instant-messaging purposes. (If you don't have one, or use another messaging service, that's alright; just indicate that here.)
E-Mail Contact: Your email address.
Time Zone: For purposes of planning events, it helps to know what time zone everyone is in.
Actors Managed: The Actors you intend to manage - you can start with up to three Actors, one of which can come from the ALT Talent Pool.
